Remove "include_in_all" field in the Elasticsearch index definition

      • all occurrences of the include_in_all settings have been removed from the Optimize index definitions

      Context:
      From Elasticsearch 6.0.0 the _all field hast been deprecated. With it also the include_in_all has been removed (see [here|https://github.com/elastic/elasticsearch/issues/22923). Since we only allow the usage of Optimize with Elastic 6.2+, we can remove those fields. You can find some examples of the usage in the process instance index definition, e.g. here
