Improve modal for entity creation

    Description

      To do:
      • Remove entity naming in the create modal completely
      • Display either 'New Report/Dashboard' as a name or the name of the Template 
      • Move blank/template selection to the Left as the first selection. 
      • Blank Dashboard has no process selection and only empty state text.
